Case Management

Case management is offered to seniors and others who have disabilities.  Such services may include assistance with Medicaid and other applications, help in finding appropriate care or care facilities, mainland transportation for medical and social-welfare appointments, and building a "safety net" for vulnerable persons who may have multiple--medical, social, or  physical--needs.  Referrals may come from persons needing help, their relatives, neighbors, and friends. 

 In addition, many people need only limited help, such as finding a good person to clean out the gutters or to clean house once or twice a month.  We maintain a list of helpers who have filled out a short form giving specific tasks they are willing and qualified to do and areas of the island in which they prefer to work.  We pass these names on to anyone who needs help; the client/requestor is responsible for checking references.
