Case management is offered to seniors and others who have disabilities. Such services may include assistance with Medicaid and other applications, help in finding appropriate care or care facilities, mainland transportation for medical and social-welfare appointments, and building a "safety net" for vulnerable persons who may have multiple--medical, social, or physical--needs. Referrals may come from persons needing help, their relatives, neighbors, and friends. In addition, many people need only limited help, such as finding a good person to clean out the gutters or to clean house once or twice a month. We maintain a list of helpers who have filled out a short form giving local references, and pass these names on to anyone who needs help; the employer is responsible for checking references.
